Matthew Kroh, MD, FACS, FASMBS, FASGE is the Vice Chair of Innovation and Technology in the Digestive Disease and Surgery Institute at Cleveland Clinic. He is the Section Head of Foregut Surgery and Surgical Endoscopy in the Department of General Surgery, and Professor of Surgery in the Cleveland Clinic Lerner College of Medicine. He most recently served as Chair, Digestive Disease Institute at Cleveland Clinic Abu Dhabi, developing clinical, operational, educational, and research programs in the United Arab Emirates. He has previously held the positions of the Program Director for the Advanced Laparoscopic Surgery and Flexible Surgical Endoscopy Fellowship at Cleveland Clinic, the Clerkship Director for Surgery in the Cleveland Clinic Lerner College of Medicine, and the Surgical Director of both the Advanced Endoscopy Center and the Esophageal Center. He has also held the role of Director of the Center for Advanced Skills Training in the Cleveland Clinic Simulation Center. He has authored more than 100 peer-reviewed articles, 30 book chapters, and 3 textbooks. He holds Committee Chair positions in the Society of American Gastrointestinal Endoscopic Surgeons and the American Society for Metabolic and Bariatric Surgery, and he is a member of the American Surgical Association. After earning a Bachelor of Science degree in biology from Boston College and a Master's degree from Boston University, Dr. Kroh received his medical degree from the Mount Sinai School of Medicine of New York University. He completed his General Surgery residency and fellowship training in Advanced Laparoscopic Surgery and Flexible Surgical Endoscopy at the Cleveland Clinic, Cleveland, Ohio, USA. Dr. Kroh is committed to clinical excellence and patient-centered care, with a particular focus on surgical and endoscopic innovation. He is licensed by the State Medical Board of Ohio and board-certified by the American Board of Surgery. His specialty interests include advanced laparoscopic surgery, bariatric surgery, gastrointestinal surgery, surgical endoscopy, endoluminal procedures, and robotic surgery.

Single-anastomosis Duodeno-ileal Bypass with Sleeve Gastrectomy (SADI-S) is an increasingly common and effective operation for treatment of obesity and weight-related co-morbidities. In addition to the technical conduct of the operation, patient selection and education for the procedure are critical to attain optimal outcomes. This lecture covers the latest data on outcomes including weight loss, improvement of metabolic disease, and complications. Operative details of the procedure, including tips and tricks as well as variations are discussed. Additional focus is placed on identifying the correct patients who will most benefit from this metabolic procedure, including indications, contra-indications, and follow-up paradigms.

Endoscopic approaches which are meant to be similar to surgery but less invasive have yielded promising preliminary results in the field of bariatrics. Treating bariatric surgical complications through the gastrointestinal lumen only is dramatically less invasive than conventional surgical revision, even if performed laparoscopically. Technological improvements have increased the variety of procedures than can be currently performed. In this lecture, Matthew Kroh from the Bariatric and Metabolic Institute at the Cleveland Clinic Ohio reviews the most recent data and outcomes regarding the endoscopic treatment of stenosis, leaks and fistula, including the revision of dilated stoma and weight recidivism.

As a result of technological improvements, the endoluminal approach takes us from a primary bariatric procedure (POSE, intragastric balloon) to the treatment of complications (gastro gastric fistulas) or revisional therapies (pouch remodeling for insufficient weight loss or weight regain). This is a rapidly growing field and procedures are being performed by either endoscopists or surgeons. As a result, there various data on short-term outcomes are available which must be carefully analyzed. Matthew Kroh of the Bariatric and Metabolic Institute at Cleveland Clinic (Ohio) is an expert in this matter and will take us through these surgical procedures performed with an endoscope.

Advanced endoscopy to treat weight regain after Roux-en-Y Gastric Bypass: Stoma dilation is the most cited cause of insufficient weight loss or weight regain after a Roux-en-Y Gastric Bypass (RYGB). Surgical revision to restore its baseline restriction characteristic may be technically demanding and associated with increased morbidity. Stoma remodeling by means of advanced endoscopic instruments represents a valid and less aggressive option. Gloria Fernandez and Oriol Sendino, from Hospital Clinic of Barcelona, were proctored by Matthew Kroh, from Cleveland Clinic Ohio, during the endoscopic revision of a dilated stoma.
