Giant Incarcerated Paraesophageal Hernia: A Robotic Repair Masterclass.

Giant incarcerated paraesophageal hernias (PEH) represent one of the most formidable challenges in upper gastrointestinal surgery. In this specialized video masterclass, Dr. Serena Guarriello demonstrates how the robotic platform can transform this complex reconstructive procedure into a highly precise and reproducible intervention. This presentation—which received the Best Video Presentation Award at the inaugural TROGSS International Congress in Luxembourg in July 2026 - offers a detailed, step-by-step review of robotic PEH repair, focusing on viscera reduction and mediastinal sac dissection. Following the surgical video, Dr. Guarriello engages in an interactive Q&A session with the TROGSS scientific panel, featuring Prof. Juan Santiago Azagra, Dr. Beniamino Pascotto, , and Dr. Rodolfo J. Oviedo. Together, they discuss critical clinical decision-making, tension-free cruroraphy techniques, and anti-reflux fundoplication choices, providing viewers with a comprehensive understanding of how to optimize clinical outcomes and minimize recurrence rates in high-risk patients.
