
Dr Silvia Secco is a full member of staff at Urologic Clinic, Niguarda Hospital in Milan, where the Retzius-sparing approach for radical robotic prostatectomy has been conceived.
She moved to Milan after graduating and finishing her residency at the University of Padua.
She has extensive surgical experience as the first surgeon in Robotic, Endoscopic and Laparoscopic Urological Surgery.
She is also passionate about functional urology and mini-invasive treatments for LUTS since the very beginning of her residency.
She is an active member in several Urological Associations (EAU, AUA, SIU, SIUD, AURO).
She is the Author of more than 50 scientific papers published in peer-reviewed impact-factored scientific journals with a Scopus H-index of 18 and more than 3000 citations.

Existing therapeutic strategies to treat LUTS due to BPH range from observation and medical treatment to a variety of surgical treatment modalities. TURP has been the treatment method of choice (even if is accompanied by a substantial perioperative morbidity rate and postoperative complications). More recently, transurethral enucleation techniques using different energy sources of laser have become the standard for larger gland sizes. There is significant interest in the development of minimally invasive surgical techniques (MISTs) for LUTS due to BPH that can be performed in an office or ambulatory setting under local anesthesia, ensure rapid and durable symptom relief, and provide a favorable safety profile (avoiding the risk of significant postoperative complications, including the development of anejaculation and/ or sexual dysfunction), as an alternative to traditional TURP or laser enucleation. In this interesting episode of AIS, we will discuss one of the most innovative and efficient MISTs available to treat LUTS due to BPH, the temporary implanted nitinol device, the so-called iTind. We will see how it works, how to properly select a patient, how and where to perform the procedure, and we will review the clinical data available.

The last decade has seen a significant shift and growth in the surgical options available in the management of symptomatic BPH. Not least driven by patients keen to take up options that avoid a general anaesthetic, that are truly ambulatory, allowing a more rapid return to normal activities but also that avoid the risk of significant post-operative complications, including the development of permanent dry ejaculation and/ or impotence.
