Surgeons will see things they will not believe in this session and will understand the huge amount of work that is going on globally to revolutionise surgery using the very best that technology can offer. One of my favourite parts of the year is watching from the podium as jaws drop in the audience during these sessions.”

The transanal approach is a revolution in treatment of rectal pathologies. The technique itself is a modification of the conventional procedure where the dissection is performed bottom- up. Upward dissection has several advantages, including objective control of the distal margin of resection and a great identification of the holy plane, even in male patients with a narrow pelvis. But this dissection also changes the characteristics of the rectal stump and the way in which it must be prepared for colorectal anastomosis. This video brings together the experience of Antonio M. de Lacy, Sam Atallah and Shady Ashamalla performing a colorectal anastomosis after a transanal total mesorectal excision. Elena Vikis will also talk about studying the functional outcomes.

When starting to apply new surgical approaches, these should be performed in a progressive and standardized manner to ensure patient safety. The transanal approach for rectal diseases is a rapidly accepted surgical procedure among colorectal surgeons, but several doubts can arise with the first cases. In this lecture, Sam Atallah, the chair of the Colon & Rectal surgery department at the Florida Hospital in Orlando, FL - USA gives his best advice to avoid complications during the taTME. This includes examining the pelvic anatomy and especially the rectal envelope in order to assess the feasibility and safety of the procedure. Also having a B plan (what to do if the taTME is not feasible) to do as much as you can from below and then complete the dissection from above. Watch this lecture for some expert tips!
