There are several benefits to the total neoadjuvant approach. It may allow for greater treatment compliance. Earlier delivery of full-dose, systemic therapy to eliminate micrometastatic disease has the potential to decrease the risk of disease progression during treatment and improve disease-related outcomes. Placing surgery as the final step in the treatment algorithm for locally advanced rectal cancer could also allow for earlier reversal of a diverting stoma postoperatively. Nevertheless, there are potential disadvantages to total neoadjuvant therapy. The delay in definitive surgery could allow local disease progression, particularly in those patients who do not respond to neoadjuvant chemotherapy. Neoadjuvant chemotherapy also may impact the performance status of patients who undergo planned surgical resection and/or potentially increase surgical complication rates.

The mesenteric organ is a single structural entity spanning the gut from duodenojejunal flexure to mesorectum. At the ileocolic level, the small intestinal mesentery continues to the right colon as the right mesocolon. A substantial mesenteric tissue mass occurs at the ileocaecal level. Anatomically, the mesentery is divisible into vascular and avascular regions. Current data indicate that local responses are mainly immunologic in nature whereas systemic changes occur in the fibrinolytic, coagulation, and inflammatory cascades. The mesentery also plays a directly protective role not unlike that of the omentum, that is, ‘the policeman’ of the peritoneum. As part of a phlegmon, it walls off and contains intraperitoneal pathology. This prevents the host mounting a generalized systemic inflammatory response to intraperitoneal disease. Mesenteric mes- othelium contributes major reparative activities such as postoperative reconstitution of mesothelial contiguity.

Clincal Case Colorectal cancer is the third most common cancer worldwide, which represents a major public health issue. About 20% of CRCs are diagnosed with acute obstruction, which is located in the right colon in one third of patients. Colonic obstruction complicates 10 to 19% of colon cancers. It is a risk factor for poor prognosis: immediate postoperative mortality ranges from 15 to 30% versus 1 to 5% for elective surgery, and the morbidity rate is twice as high as in an elective setting.

The term "anal cancer" usually refers to a squamous cell cancer (SCC) of the anal canal, which represents the majority of cancers arising in the anal region. However, SCCs can also arise in the perianal skin and in the rectum, and SCCs arising at these sites are generally treated in the same manner as anal canal SCCs. This topic review will cover the treatment of SCC and what´s new about that.

Colorectal Cancer (CRC) is a common and lethal disease. Its incidence and mortality rates vary markedly around the world. Globally, CRC is the third most commonly diagnosed cancer in males and the second in females, with 1.8 million new cases and almost 861,000 deaths in 2018 according to the World Health Organization. But, What about México? Mexico has a population of more than 130 million people. In 2018, 190.667 new cases of cancer were diagnosed, where the CRC accounted for 7.8% of the cases. Mortality in Mexico due to CRC is 6.8%. The decline in CRC mortality going to start after widespread and well implementation of CRC screening and the effective adjuvant therapy become properly use.
