
David is Bowel Cancer UK & RCS Engl. Professor of Surgery at the University of Leeds and Hon. Consultant Surgeon at Leeds Teaching Hospitals NHS Trust. His clinical interests include robotic and minimally invasive surgery for colorectal cancer and pelvic floor dysfunction. His research interests include the development of new surgical technologies and devices to improve outcomes in colorectal disease. In 2012 he was awarded an NIHR Research Professorship to accelerate research and implementation of new technologies into clinical practice. He is currently an NIHR Senior Investigator. He is Chief Investigator for several NIHR portfolio clinical trials. He has previously served on the EME/NIHR Prioritisation and Strategy Groups, the Doctoral Research Fellowship Committee, and the Clinician Scientist Awards Panel. He is currently a member of the NIHR Advanced Fellowship Panel and the NIHR i4i Programmes. He is Clinical Director of the NIHR MedTech Co-operative in Surgical Technologies, a national network of clinicians, academics, patient & public representatives, and commercial partners to develop novel solutions to unmet surgical need. His position as Clinical Director for the NIHT Global health Research Group affords him the opportunity to apply his expertise in the clinical translation of surgical technologies to the context of low resource countries.

Dr. Jayne presented the results of the FIAT trial (Fistula In Ano Trial), which involved 50 centers within the UK randomizing 304 patients for the fistula plug versus other surgical treatments for fistula over a 5-year period. The surgical procedures performed were fistulotomy, cutting seton, LIFT, and advancement flap. Approximately 50% of patients had undergone previous fistula treatments. A total of 152 patients were allocated to each arm. The primary outcomes were the fecal incontinence and quality of life scores, with a low rate of incontinence and overall similar outcomes in both groups. There were no statistical significant differences on fistula healing between arms, as well as for complications, except for pain, which was significantly higher with the fistula plug. Extrusion remains a concern for this procedure, with up to 15%. Re-interventions are very common, with approximately 50% of further intervention in 12 months. More data should be collected regarding the economics and long-term follow-up of patients. [Image]

Despite its short history, AIS Channel has become the main global platform for surgical teaching. Currently focusing on colorectal, bariatric, and endocrine surgery, it will expand to new surgical specialities in 2017 with the goal of encompassing all surgical specialities. In this video, we will see some members of the AIS Board give their views in this regard, emphasising the huge usefulness of this technological revolution which AIS Channel has brought about in the surgical world. No doubt, part of AIS’s success is based in having the best surgeons in the world as its teachers, enabling the global surgical community to access them. This is a huge added value that joins the long list of services and opportunities offered by AIS, always in order to boost the professional development of the entire AIS community as much as possible.

One of the main goals of AIS Channel is to identify and broadcast technological innovations that improve surgical outcomes. This year, our summer event was held on July 14th: a live streaming congress that focused on oncologic assessment with fluorescence imaging. Steven D Wexner designed a robust agenda that included the most relevant data aimed at enhancing lymph node assessment prior to, during and after surgery. This will potentially lead to a more precise and tailored treatment. Experts from the United States, the United Kingdom, Spain, Ireland and Switzerland brought the latest data, exchanged ideas, and answered the questions sent by viewers from all over the world, providing a global opinion. Simultaneous Live Surgeries were performed by Antonio Lacy, Raquel Bravo & María Fernández-Hevia, using the laparoscopic and robotic approach for treatment of rectal cancer. This was an exciting experience moderated by David Jayne. During the procedures, several ICG guided strategies were used for the lymphovascular dissection, tissue assessment, and anastomotic perfusion. Both resected specimens were evaluated by Mariana Berho, who confirmed a correct dissection. Finally all the experts gave their final statements and closing remarks which you may see in this video. The 2016 AIS Summer Event was once again a huge success in terms of audience. Thousands and thousands of colorectal surgeons from 99 countries logged in and watched the entire event live. Once again, AIS Channel is consolidating itself as the most innovative and popular digital tool in surgical education, through its approach of providing the entire global medical community with the best training.
