PROGRAM 1. SSI in OPEN colorectal surgery: right hemicolectomy - Dr. Woramin Riansuwan Information is very important to understand what we are dealing with. So data about the costs and amount of incidence of Surgical Site Infections (SSIs) is a clear indicator of how important the treatment of this complication is. In order to find solutions, it is crucial to know the 3 types of SSI: Superficial Surgical Site Infections, Deep Surgical Site Infections and Organ Space Surgical Site Infection; and the 4 different types of prevention: pre- hospitalization, preoperative, intraoperative and postoperative. Although there are different methods of prevention, some of them are more common in the majority of operations, which are explained in more detail. Finally, for specific operations, more useful methods can be applied to prevent SSI. In the case of open right colectomy, specific methods exist too. These are covered by instructional videos, and some final recommendations are given. 2. LAR for colorectal cancer. Recommendations for SSI prevention - Dr. Brendan Moran As is well known, there are continuous developments in medicine to improve techniques and patients outcomes. That is why continuous advances are being made for every illness. Dr Moran discusses the advances in the management of rectal cancer, arguing that selecting the best approach is more important than the incisions. The use of MRI (Magnetic Resonance Imaging) in rectal cancer could help in the decision-making. He then shows all the steps of TME (Total Mesorectal Excision), including patient examinations and the conditions that the surgeon should be aware of: preoperative planning, the techniques for the procedure and, finally, the conclusions. The different techniques are described using videos or images for clarity. 3. SSI in open colorectal surgery: left colectomy - Dr. Luis Chiroque The topic of Surgical Site Infection (SSI) is introduced by a discussion of colorectal cancer in Peru. The question is how to prevent this problem in the specific case of open left colectomy. Dr. Chiroque discusses risk factors and various methods, procedures, and techniques to prevent SSI by means of visual help such as photos and videos. More than 120 elective colorectal procedures are performed every year, 85% of which are oncological cases according to data from Almenara Hospital in Lima. The main causes of bowel obstruction are sigmoid volvulus, followed by hernias and colon cancer. Bearing in mind the safety standards, the usual approach taken is laparoscopic and open colorectal resection.

First, the past, present and future of the total mesorectal excision were discussed by its creator, Richard Heald. Then a series of lectures given by Mariana Berho, Paris Tekkis and Steven Wexner focused on hot topics: Do the results of TME vary by technique?, Do the clinical outcomes vary with technique? and Attempts to reduce anastomotic leaks. A block of videos was broadcast where the surgical approaches to TME were evaluated. Brendan Moran talked about the open approach and its place in current practice. Yves Panis talked about the laparoscopic TME and its advantages. Jim Khan was in charge of demonstrating the robotic TME, showing how an accurate dissection was performed. And Antonio Lacy explained all the essentials, tips and tricks to perform a taTME, which is the most innovative and least invasive approach. The most interesting part of the event was having great surgeons perform simultaneous and live open, laparoscopic, robotic and transanal TME procedures. This dynamics made it possible to compare the benefits, limitations and pitfalls of the current approaches in front of a huge audience of surgeons from all over the world. It was an amazing experience. Once the specimens had been resected the quality of the dissection was assessed by Mariana Berho and Nicholas West. Finally the panel of experts held a discussion, giving their views and recommendations. Regarding the statistics of the event, the AIS Channel Winter Event 2016 was again hugely successful. It was seen live by more than 25,000 viewers from 102 countries. Interaction with the global audience was once again very powerful. Moreover, these amazing figures will increase once the event is published on AIS for permanent viewing in the next few days.
