Dr. Antonio Caycedo-Marulanda is the Chief of Colorectal Surgery at Orlando Health in Orlando, Florida, where he also serves as the institution’s Medical Director for the National Accreditation Program for Rectal Cancer (NAPRC). In this role, he leads the development, implementation, and oversight of standardized rectal cancer care pathways across the health system. He is also an Associate Professor of Surgery at the University of Central Florida, with ongoing academic and research affiliations in Canada as an Adjunct Associate Professor of Surgery at Queen’s University and Associate Clinical Researcher at the Health Sciences North Research Institute (HSNRI).
Originally from Colombia, Dr. Caycedo earned his medical degree from Universidad El Bosque and completed his general surgery residency at Javeriana University in Bogotá. He relocated to Canada in 2004, retrained in general surgery at the University of Ottawa, and subsequently completed a colorectal surgery fellowship at Western University in London, Ontario. He later earned a Master’s in Clinical Research from the University of Liverpool (UK) and is a graduate of the 2023–2024 Surgical Leadership Program at Harvard Medical School Postgraduate Medical Education.
He began his Canadian career by founding the colorectal surgery program at Health Sciences North (HSN) in Northern Ontario. There, he became the first surgeon in Canada to introduce transanal minimally invasive surgery (TAMIS) and also pioneered transanal total mesorectal excision (taTME) in the country. He led the implementation of the institution’s first Enhanced Recovery After Surgery (ERAS) protocol. After nearly a decade at HSN, he joined Kingston Health Sciences Centre and Queen’s University, contributing to one of Canada’s highest-volume robotic colorectal surgery programs.
Dr. Caycedo is a Fellow of the Royal College of Surgeons of Canada, the Canadian Society of Colon and Rectal Surgeons, the American College of Surgeons, and the American Society of Colon and Rectal Surgeons (ASCRS). He serves on the New Technologies Committees of both SAGES and ASCRS, and previously held the role of Regional Surgical Oncology Lead for Cancer Care Ontario. He is also an examiner for the Royal College of Surgeons of Canada.
A strong advocate of clinical innovation and international collaboration, he contributed to the global consensus on the safe implementation of taTME, founded the Canadian taTME Expert Collaboration (CaTaCO), and has participated in the COLOR III trial and the international taTME registry. He also co-authored the Pan-Canadian Rectal Cancer Guidelines.
Dr. Caycedo has authored over 75 peer-reviewed publications, contributed to five book chapters, and serves on the editorial board of several surgical journals. He lectures nationally and internationally, with clinical and academic interests in rectal cancer surgery, advanced minimally invasive and robotic techniques, and environmentally sustainable surgical practices.

Dr. Antonio Caycedo-Marulanda is the Chief of Colorectal Surgery at Orlando Health in Orlando, Florida, where he also serves as the institution’s Medical Director for the National Accreditation Program for Rectal Cancer (NAPRC). In this role, he leads the development, implementation, and oversight of standardized rectal cancer care pathways across the health system. He is also an Associate Professor of Surgery at the University of Central Florida, with ongoing academic and research affiliations in Canada as an Adjunct Associate Professor of Surgery at Queen’s University and Associate Clinical Researcher at the Health Sciences North Research Institute (HSNRI). Originally from Colombia, Dr. Caycedo earned his medical degree from Universidad El Bosque and completed his general surgery residency at Javeriana University in Bogotá. He relocated to Canada in 2004, retrained in general surgery at the University of Ottawa, and subsequently completed a colorectal surgery fellowship at Western University in London, Ontario. He later earned a Master’s in Clinical Research from the University of Liverpool (UK) and is a graduate of the 2023–2024 Surgical Leadership Program at Harvard Medical School Postgraduate Medical Education. He began his Canadian career by founding the colorectal surgery program at Health Sciences North (HSN) in Northern Ontario. There, he became the first surgeon in Canada to introduce transanal minimally invasive surgery (TAMIS) and also pioneered transanal total mesorectal excision (taTME) in the country. He led the implementation of the institution’s first Enhanced Recovery After Surgery (ERAS) protocol. After nearly a decade at HSN, he joined Kingston Health Sciences Centre and Queen’s University, contributing to one of Canada’s highest-volume robotic colorectal surgery programs. Dr. Caycedo is a Fellow of the Royal College of Surgeons of Canada, the Canadian Society of Colon and Rectal Surgeons, the American College of Surgeons, and the American Society of Colon and Rectal Surgeons (ASCRS). He serves on the New Technologies Committees of both SAGES and ASCRS, and previously held the role of Regional Surgical Oncology Lead for Cancer Care Ontario. He is also an examiner for the Royal College of Surgeons of Canada. A strong advocate of clinical innovation and international collaboration, he contributed to the global consensus on the safe implementation of taTME, founded the Canadian taTME Expert Collaboration (CaTaCO), and has participated in the COLOR III trial and the international taTME registry. He also co-authored the Pan-Canadian Rectal Cancer Guidelines. Dr. Caycedo has authored over 75 peer-reviewed publications, contributed to five book chapters, and serves on the editorial board of several surgical journals. He lectures nationally and internationally, with clinical and academic interests in rectal cancer surgery, advanced minimally invasive and robotic techniques, and environmentally sustainable surgical practices.

This event has the main objective of advising how to manage cancer through transanal minimally invasive surgery (TAMIS). Dr. Antonio Caycedo Marulanda will discuss his experience and outcomes in performing TAMIS for the management of benign large rectal polyps and early malignant rectal cancers. Dr. Caycedo's study is based on the first Canadian center to adopt the technique and includes indications such as a lesion diameter of less than 30% of the circumference and T1 with a benign lesion. However, contraindications include positive lymph nodes, distant metastasis, and ulcerated tumors. The importance of magnetic resonance imaging (MRI) for the management of patients with rectal villous adenoma will also be discussed. One crucial aspect that will be addressed is the surgical defect after transanal endoscopic microsurgery (TEM): open versus closed management. This is an important consideration for achieving optimal outcomes, minimizing complications, and promoting faster recovery. Lastly, the NEO trial will also be discussed, evaluating the use of neoadjuvant chemotherapy and radiation therapy in combination with TAMIS for patients with locally advanced rectal cancer.

Bleeding before, during or after surgery can significantly compromise the success of surgical procedures and consequently the safety of patients. A large percentage of individuals who undergo surgery have either a form of bleeding disorder or require anticoagulant medication impacting their underlying risk of bleeding. As colorectal surgeons, we must make significant considerations, including assessing the risk of bleeding associated with the specific procedure, timing of interruption and resumption of anticoagulation therapy, and whether to use bridging therapy. Management of intraoperative bleeding can be divided between pharmaceutical (sealants) vs surgical maneuvers. Postoperative bleeding requires prompt identification and management, in the form of further surgery or the involvement of interventional radiology. It is important to understand the basics of a topic before starting to think of new solutions. In this case, antimicrobial resistance, which makes bacteria more difficult to treat. Since the discovery of the first one (penicillin), antimicrobials have been used for many medical interventions that involve low morbidity. However, antimicrobials not only have a positive effect, but also they generate a reaction in the bacteria that induce other bacteria to be prepared to defeat the effects of the drug, making them stronger and resistant. The problems are the misuse and overuse of these medications and the failure to discover new ones, which make the situation more dangerous. Dr. Caycedo-Marulanda discusses provisional data on the future situation, demonstrating the need for new solutions due to the increasing urgency of a change in current medical treatment. Given the evidence of antibiotic resistance, a new solution plan is needed to avoid complete resistance of bacteria, as the existing 8-goal plan is not yet successful. He ends the video by making recommendations on the procedure to avoid the need for antibiotics and help reduce antimicrobial resistance; as well as the principles of a good antibiotic treatment.

An anastomosis is defined as a surgical connection between adjacent channels. Typically, for right hemicolectomy, an anastomosis is created between the transverse colon and the terminal ileum after exteriorization and subsequent division of the bowel. This requires traction and manipulation of the segment involved which ultimately increases the impact of the surgical insult. Laparoscopic surgery has been the epitome of MIS over the last 4 decades, however its basic principles are usually breached during the reconstructive phase of right hemicolectomies. An intracorporeal anastomosis in this type of cases has many potential advantages that include: Lesser surgical insult Decreased length of stay Fewer hernias Potential shorter incision Decreased risk of bleeding and serosa tears There is evidence supporting this: the existing RCT demonstrated faster recovery with no effect on LOSS. However, many cultural and personal factor in surgeons and patients have an impact. Technical aspects include: Positioning: Supine in a secured fashion (able to tolerate Trendelenburg and lateral tilt positions. Surgical ports are placed as depicted. Regular laparoscopic equipment is used, including linear endo GI staplers and a barbed 3-0 suture. The video deshowmonstrates the steps. Mesenteric division x 2 Colon and small bowel (Harmonic ® scalpel) Colonic transection.
